We are pleased to announce a new, exciting and most importantly, free service to all of our debit card customers. Protecting you from unauthorized use of your debit card is one of our top priorities at F&M Bank. To mitigate fraud, we have partnered with SHAZAM®, our electronic funds transfer network, to implement a monitoring system, called SHAZAM® R.A.D.A.R., to help guard your debit card against fraudulent activity. If suspicious activity is detected on your debit card, you will be contacted by F&M Bank or by a SHAZAM® fraud specialist calling on our behalf, to verify the transactions in question.
To ensure that we can reach you promptly if fraudulent activity is suspected, we need to have current contact information on file. Of course, we already have your name, address and similar data on file. However, with the growing prevalence of cell phones and changes in phone numbers, we want to make sure that we have your current telephone information on file.

In the past five years, if you have:
• Changed your home telephone number;
• Changed your daytime or work telephone number;
• Started using a cell phone; or
• Changed your existing cell phone provider or cell phone number
Please contact our Main Bank at (309) 343-7141, from 8 a.m. to 5 p.m. CST Monday through Friday to help ensure that we have the correct information or fill out the enclosed information card and drop off at any F&M Bank location.

We encourage you to provide both a primary and secondary phone number to help us avoid any delays in response to fraud alerts. These may be your home, work, or cell phone numbers. It is very important that we keep this information current, so please contact us promptly if it ever changes in the future. Also, if you should ever travel outside of the U.S., please let us know in advance; it will help us to protect your account.
As is the case with all of your personal information, we will keep this data strictly confidential. Please remember that we will not ask for your debit card number or personal identification number (PIN) to verify your identity. Always use caution when providing your debit card information and contact us immediately if you should ever suspect that your debit card has been stolen or compromised.
Enrollment in the SHAZAM® R.A.D.A.R. program is being provided at no cost to all of our debit card customers. It’s just one more way that we put your safety and security first here at F&M Bank!
